Submit a Referral

Before you send a referral, you will need your unique clinic login information. Please feel free to utilize the "Username Help?" and "Password Help?" links on the login page to obtain your information -OR- feel free to contact us via phone or email for this information.

Follow the prompts on the referral form. You have the option of attaching records or emailing them separately. You will receive a copy of the referral form via email upon completion. We ask that forms are filled out for emergency referrals after they are called in.

Feel free to check in on patients through our portal in real time! Once a case is completed, you will receive an email with a link to the most up to date medical record, in which you can also find in the portal.
